Social Prescribing Link Workers (SPLW)
Social prescribing is a service designed to support people with a wide range of social, emotional or practical needs with the focus on improving mental health and physical well-being.
Social Prescribing Link Workers (SPLWs) provide time to talk and explore what matters to a person, and will help them to create a wellbeing plan with attainable goals.
They then link people to a wide range of non-clinical groups, services and activities which are typically provided by voluntary and community sector organisations.
To refer yourself to a Social Prescriber, you can speak with your GP surgery who can book you an appointment.
